Any item or material that does not meet the commercial standard or procurement requirements as defined in such sources as catalogues, proposals, procurement specifications, design specifications, testing requirements, or contracts. It does not include parts or services that fail or are otherwise found to be inadequate because of random failures or errors within the accepted reliability level.
