Accident

The term "accident" can be defined as an unplanned event that has a resulted in or suggests the failure of a DOE safety management system, barriers, or loss of controls that rises to the threshold criteria specified in this Appendix A of DOE O 225.1B.

An unplanned sequence of events that results in undesirable consequences.

For the purposes of answering USQD questions, an event or sequence of events that have the potential to result in undesirable consequences.
