The document references DOE standards such as DOE-STD-1197-2011 and DOE STD-1098-2008 but does not provide links to the standards. A search on the standard using the page's search feature only retrieves the directive.

For 4A(1) and 4A(2) Responses to Frequently Asked Questions the following was asked: If there is a temporary loss, but all actions and completions times are met and there was no compromise to the authorization bases, is this reportable? In both bases the response was "Yes." If the question had been the following would the answer still have been yes:

If there is a temporary loss of a component where redundant components are maintained operable or in operation and the
authorization basis is not compromised, is this reportable? (Based on definition of Performance Degradation - item (3)) It would appear that having a redundant operable component would not make it reportable, even if a LCO had been entered.
